Nina — The Hospital Story

Spoilers ahead

This guide explains Nina's hospital storyline and its family milestones. Intimate events are summarized without graphic detail.

Nina is the maternity charge nurse at Woodchester Hospital. Her story develops through the deliveries she attends: professional banter gives way to a more personal conversation about the family she wants for herself.

Story availability

Nina's hospital storyline is not available in the free build.

🏁 How Nina's story progresses

Nina has three numbered milestone conversations, followed by a 4-0 objective completed through her normal hospital interaction. Pregnancy and the post-birth visit extend that story afterward; they are condition-based follow-ups, not missing numbered chapters.

Stage Requirement How it starts
1-0 First qualifying birth Automatically after the delivery scene finishes.
2-0 Children with two different qualifying mothers Automatically after a birth reaches that milestone.
3-0 Children with three different qualifying mothers Automatically after the third-mother milestone.
4-0 Nina has asked for a child; first-visit preparation is complete Visit Nina at the Hospital and use her new relationship option.
Pregnancy Nina's pregnancy begins Follow her messages and visit the Hospital.
Birth and afterward Nina reaches delivery day Visit and speak to her at the Hospital, then continue her regular visits.

The milestone is different mothers, not total babies. Twins, triplets, or another child with the same mother do not substitute for the second or third mother. Fiona's births do not count toward Nina's storyline because Fiona does not deliver at the Hospital.

🏥 Before Chapter 1 — Meeting the nurse

You can meet Nina during a normal visit to the Hospital before her birth milestones begin. Talk to her when she is available at the nurses' station to learn about her job and her life away from the ward.

This introductory conversation is optional preparation. Meeting her early changes how she recognizes Ethan after the first delivery, but repeatedly selecting her everyday conversation topics does not replace the family milestones.

Next: Progress another qualifying character's family story through the birth of a child.

Chapter 1-0 — The first delivery

Requirement: A first child with a qualifying character. Trigger: The follow-up plays automatically once the birth scene has finished and normal play resumes.

Nina checks in on Ethan after the delivery and reassures him that mother and baby are doing well. She gives him a humorous assessment of how he handled the occasion, talks about her work on the ward, and offers advice for a new parent.

The conversation also introduces a quieter side of Nina: she loves her work, but every baby she helps deliver goes home with somebody else. Whether Ethan met her beforehand affects the greeting, not the next milestone.

Next: Have a child with a second different qualifying mother. Another birth with the first mother will not advance this chapter.

Chapter 2-0 — A more personal conversation

Requirement: Children with two different qualifying mothers. Trigger: Automatically after a qualifying delivery.

Nina notices Ethan's name appearing on more than one family's paperwork. Her teasing becomes a discussion of her own past, including an engagement that ended after years of postponing the family she wanted.

Ethan can respond sympathetically or begin to ask more directly about what she wants. Finish the conversation; there is no item to buy or separate hospital appointment to book at this point.

Next: Reach a birth with a third different qualifying mother. The number of children already born to the first two does not change this requirement.

Chapter 3-0 — Nina makes her decision

Requirement: Children with three different qualifying mothers. Trigger: Automatically after the milestone delivery.

Nina waits to talk to Ethan after her shift. She explains how seeing him return for each family has changed her view of him and finally asks to start a family together.

The response options change the tone of the exchange. The story then gives the preparation instructions for the next visit and opens the new relationship option in her regular hospital conversation.

Next: Prepare for 4-0. Visiting her immediately without completing the first-visit requirements can end with her sending Ethan away to finish them.

Chapter 4-0 — The first appointment

Prepare before going to the Hospital

Nina's first appointment has two in-game requirements:

  1. Let four full in-game days pass without Ethan finishing an intimate activity, whether alone or with another character.
  2. Use a Sperm Booster so its timed effect is active when you arrive. Owning one in the inventory does not activate it.

Check the game's odds display for the current chance of a pregnancy. Completing the appointment requirements does not guarantee pregnancy.

Speak to Nina

Visit her at the Hospital and select Something More.... The newly available family option starts the commitment conversation, then the appointment. Finish that interaction to continue into her repeatable relationship content.

If either preparation condition is missing on the first visit, Nina explains what is missing and sends Ethan away. Complete that condition and return; there is no need to replay the three birth-milestone chapters.

Further attempts

After the first completed appointment, you can return without repeating the full preparation. Nina's next major family step still depends on an actual pregnancy; check the pregnancy system for the result of each visit.

Pregnancy — Messages and hospital visits

The announcement

When Nina's pregnancy begins, follow her phone message and keep checking her entry in the pregnancy system. This is a new state of her relationship, not a fourth different-mother milestone.

Next: Continue normal days and visit her at the Hospital as the pregnancy advances.

The changing conversations

Nina's hospital interaction includes updates as her pregnancy progresses. Her practical nurse persona remains, but the conversations now concern her own family. Return after progression rather than expecting every pregnancy conversation to appear during a single visit.

Use the pregnancy tracker to see Nina's current progress and delivery timing.

Delivery day

When Nina is ready to give birth, go to the Hospital and speak to her at the station. Her delivery is handled through her own interaction. Simply wandering through the lobby is not the same as completing that conversation.

Finish the delivery and reunion sequence to complete this pregnancy. The family record then reflects the new child or children.

After the birth

Return to Nina at the Hospital for her regular conversation and relationship options. Her work-related dialogue also responds to how many children are in the daycare, giving the ward a different feel as the wider family grows.

Further family play is repeatable content. It does not restart 1-0, and it does not require three additional mothers before Nina can continue her own relationship.

💡 If you are stuck

Problem What to check
Nina's story is absent Her hospital storyline is not available in the free build.
Another birth did not advance her story Count different mothers. Repeated births and multiples do not create another mother milestone. Fiona is excluded.
The birth just finished but Nina has not spoken Finish closing the active scene and allow normal play to resume so the queued conversation can start.
Something More... is unavailable Complete 3-0, where Nina asks for a child.
Nina turns Ethan away Complete four full days of preparation and activate a Booster before the first appointment.
The appointment happened but no pregnancy began Check the actual result in the game's pregnancy system; the appointment is not a guaranteed conception.
Nina is due but delivery will not start Find and talk to Nina at the Hospital.
